I acknowledge that Raging Thunder is relying on this information in determining my suitability both to undertake the training course and if I successfully complete that course, for employment as a commercial white water rafting guide.

I understand that Raging Thunder reserves the right to exclude me from further participation in the course at any time if my performance or behaviour is unsatisfactory.
